How to Create a SharePoint Survey in Under 5 Minutes” explains how to quickly create a survey in SharePoint using only out of the box functionality.

The first step is to make sure that the “Team Collaboration Lists” Feature is turned on by going to Site Settings -> Site Features and clicking “Activate” under Team Collaboration Lists if it is not already Active.  Then go to “View All Site Content” and the “Create” menu, and select “Survey” list and name your list.  Then create questions and answer types.  You could stop there, but if you would like things like “Branching Logic” to the answers you can configure that, or you could set who can view and edit answers or opt out of search crawling.

Thats it! All you need to do now is ask your users to fill in the survey and voila!
